Please use the form below to order your repeat prescription.

Prescriptions are typically issued during an in person consultation and a plan is put in place for repeat prescription requests. If it has been more than 6 months since you have consulted your doctor or you are taking medications that require blood-test monitoring or a blood pressure check, we may advise you by text or email that a consultation is necessary before your repeat prescription can be issued safely.

THE PROCESS

This form is for repeat prescription requests only, any new medication will need to be discussed during a consultation. Prescriptions will be sent directly to the pharmacy of your choosing.

Please indicate which pharmacy you would like us to send your prescription to (if not listed let us know and we can add it).

Please allow 48 hours for us to review and process your prescription request.

There is a €30 fee, paid in advance of the prescription being sent, the payment link will be sent securely to your phone or to pay over the phone, please call the practice.
